Hyperosmolar therapy for regulation of cerebral edema and intracranial pressure

چکیده

Background: Hyperosmolar therapy is an essential treatment method for increased intracranial pressure and cerebral edema. Mannitol hypertonic saline are frequently used in clinical practice; however, more helpful recommendations needed the optimal management of edema terms choice, dosage, timing these medications. This study aimed to introduce characteristics relative strengths two agents, i.e., mannitol saline, review data supporting their use various diseases.Current Concepts: reduces by removing water from brain tissue transferring it vascular space creating osmotic gradient. improves blood flow reducing hematocrit, decreasing viscosity, increasing deformability red cells. Hypertonic increases intravascular volume, transiently cardiac output, oxygen partial brain. has several advantages over mannitol, including quicker onset longer-lasting reduction pressure. However, no significant differences noted clinical, functional outcomes, or mortality between agents.Discussion Conclusion: Both effective Clinicians should be able select appropriate agent different situations based on available evidence patients’ individual medical conditions.

Hyperosmolar therapy for intracranial hypertension.

The use of hyperosmolar agents for intracranial hypertension was introduced in the early 20th century and remains a mainstay of therapy for patients with cerebral edema. Both animal and human studies have demonstrated the efficacy of two hyperosmolar agents, mannitol and hypertonic saline, in reducing intracranial pressure via volume redistribution, plasma expansion, rheologic modifications, an...

Hypertonic saline for cerebral edema and elevated intracranial pressure.

The use of HS solutions has been shown to reduce ICP both in animal models and in human studies in a variety of underlying disorders, even in cases refractory to treatment with hyperventilation and mannitol. There are several possible mechanisms of action, and important complications such as central pontine myelinolysis and intracranial hemorrhage have not been reported in the human studies. Di...

Intracranial pressure gradients caused by experimental cerebral ischemia and edema.

Intracranial Pressure Gradients Caused by Experimental Cerebral Ischemia and Edema • To assess the development and resolution of ischemic cerebral edema and brain swelling, measurements of intracranial pressure (ICP) were made in cats after the transorbital occlusion of one middle cerebral artery. Measurement of ICP gradients was difficult: epidural, subdural and intraventricular catheters caus...

Evidence for cerebral edema, cerebral perfusion, and intracranial pressure elevations in acute mountain sickness.

INTRODUCTION We hypothesized that cerebral alterations in edema, perfusion, and/or intracranial pressure (ICP) are related to the development of acute mountain sickness (AMS). METHODS To vary AMS, we manipulated ambient oxygen, barometric pressure, and exercise duration. Thirty-six subjects were tested before, during and after 8 h exposures in (1) normobaric normoxia (NN; 300 m elevation equi...
